COD4 v1.4 Patch:

If you purchased the game via a digital distribution company (Steam, Direct 2 Drive etc). Then you must wait for the new patch to be released through them OR you can fiddle with the registry to make the retail patch work.

NOTE: If you cock up the computer by faffing with the registry then don't blame me.

Goto the 'run' from within the start menu and type 'regedit'.

In the left panel browse to 'HKEY_LOCAL_MACHINE / SOFTWARE / Activision / Call of Duty 4'.

A list of variables to appear in the right hand panel relating to the game.

Find the one called 'Digital Distribution' and rename 'Digital Distribution' to anything else.

Minimise the Registry Editor (You will need it again in a second) and install the retail patch.

Once installed, rename the once called 'Digital Distribution' back to 'Digital Distribution'.

Play the game.

I must point out that this is just a temporary fix for the impatient. Keep checking if your game provider has released a proper patch for their distribution and install it when it's available.
